Once you have booked your course, you will need to complete the forms below.

Do not post these forms. Bring them with you on the first day of your course.

Everyone needs to sign a Self-Declaration of Fitness to Parachute form before they can skydive.  If you have any of the listed medical conditions then you will need to request medical advice by taking the Medical Form to a Doctor.

If you are under 18 years old, this form will need to be signed by your parent or guardian. 

British Skydiving Self-Declaration Form - 115C

Medical Form

If you have pre-existing medical conditions, take this to a doctor for medical advice.

Student Provisional Membership & Under 18's Agreement

This form needs to ber compelted by everyone.  However, if you are under 18, you will need a parent or guardian to sign this form.  Please bring it with you on the first day of your course.
