Do not post these forms. Bring them with you on the first day of your course.
Everyone needs to sign a Self-Declaration of Fitness to Parachute form before they can skydive. If you have any of the listed medical conditions then you will need to request medical advice by taking the Medical Form to a Doctor.
If you have booked a course, you will find all the details you need in this confirmation pack, If you do not have either of the following forms you will not be able to jump.
If you are under 18 years old, this form will need to be signed by your parent or guardian.
If you have pre-existing medical conditions, take this to a doctor for medical advice.
Student Provisional Membership & Under 18's Agreement
This form needs to ber compelted by everyone. However, if you are under 18, you will need a parent or guardian to sign this form. Please bring it with you on the first day of your course.
Guest Pro Forma
Please complete this form with details of all members in your party and bring an up to date copy with you each day of your course. You will be asked for this form at the gates to Netheravon Airfield. Don't forget photo ID for everyone age 16 and over (for example a driver's licence or passport).
